Application FormBy filling out this income & expense report, you are providing your Licensed Trustee with the financial information needed to prepare the legal documents for either a bankruptcy or consumer proposal. If you file for personal bankruptcy, you will also be required to fill this report out each month.

If you have more than one source of self-employed income, separating your different kinds of self-employment income and expenses by type is a great place to start.
